Form 1095-A (Health Insurance Marketplace Statement) is sent by the federal or state Marketplace to anyone who enrolled in a Marketplace plan. It shows the months you were covered, the premium amount, and any Advance Premium Tax Credits (APTC) paid on your behalf. You must use Form 1095-A to complete Form 8962 when filing your federal tax return — this reconciles the advance credits you received with the actual credit you qualify for based on your final income. If your income was higher than estimated, you may owe money back. If lower, you may receive a refund.

Real-World Example

You enrolled in a Silver marketplace plan and received $300/month in advance premium tax credits. At tax time, you receive Form 1095-A. When you file your return, you discover your income was slightly higher than estimated — you reconcile and repay $400 of the credits received.
