Form 1095-B is issued by insurance providers (private insurers, Medicaid, CHIP, Medicare Part A) to confirm that you and your dependents had minimum essential coverage (MEC) during the tax year. While the federal individual mandate penalty was eliminated in 2019, some states still require proof of coverage. The 1095-B lists covered individuals and the months of coverage. You do not need to attach it to your federal tax return, but you should keep it for your records. Employers with self-insured plans also issue 1095-B forms.

Real-World Example

You were covered by Medicaid for 10 months of the year. Your state Medicaid agency sends you a 1095-B in January showing coverage for those months. Your state has an individual mandate, so you use this form to prove coverage when filing your state return.
