EPO (Exclusive Provider Organization)
A plan with no referrals required but no out-of-network coverage except emergencies.
Full Definition
An EPO combines aspects of HMOs and PPOs. Like a PPO, you don't need referrals to see specialists. Like an HMO, you must use the plan's network — out-of-network care is not covered except for true emergencies. EPOs often have moderate premiums.
Real-World Example
With your EPO, you can see a specialist directly. But if you travel and visit an out-of-network urgent care clinic for a sprained ankle, you'll pay the full bill yourself.
