An HMO requires you to choose a primary care physician (PCP) who coordinates your care. You must get a referral from your PCP to see a specialist. Out-of-network care is generally not covered except in emergencies. HMOs tend to have lower premiums and predictable copays, but less flexibility in choosing providers.

Real-World Example

Your HMO plan's PCP is Dr. Smith. To see a cardiologist, you first visit Dr. Smith, who writes a referral. Seeing a cardiologist directly without a referral means you pay the full cost yourself.
