POS Plan (Point of Service)

A POS plan is a hybrid of HMO and PPO. You need a primary care physician and referrals for specialists, like an HMO. But unlike an HMO, you can see out-of-network providers — you just pay more for doing so. POS plans offer a middle ground between cost savings and provider flexibility.
Real-World Example
Your POS plan's PCP gives you a referral for a physical therapist. You choose an out-of-network PT — you still get some coverage, but your coinsurance is higher than if you'd chosen an in-network PT.
