In-network providers have negotiated contracts with your health insurance plan to provide services at pre-agreed rates. When you use in-network providers, your cost-sharing (deductibles, copays, coinsurance) is significantly lower. Your insurance company typically maintains a directory of in-network providers. HMO and EPO plans only cover in-network care (except emergencies), while PPO and POS plans offer partial out-of-network coverage.

Your plan lists Dr. Patel as an in-network primary care physician. A visit costs you only a $30 copay. Seeing an out-of-network doctor could cost you hundreds of dollars out of pocket.
